THE APPLICATION AND
AGREEMENT POLICY
This application acknowledges his or her sole responsibility for physical and mental conditioning and health, and hereby indemnifies and holds harmless Instructor Chuck Wayne said owner, employees, and all assistant training Instructors, against any and all claims or demands for injuries or damages to the applicant resulting or arising out of the course of instruction contracted for, hereby and all practices, examinations and contest associated or connected therewith. This class is licensed owner and operated by Chuck Wayne and no other person's, members, or sponsors of this program are responsible for conducting classes, demonstrations, opening schools under Box Hop Ju 5ul, and setting up tournament. Any other person's or student is not permitted to teach any part of this program without permission from the Instructor Chuck Wayne or the Box Hop Ju Sul Association. All instructors of Box Hop Ju Sul must be promoted to black belt 5th level before conducting their own school. Unless otherwise instructed by Master Chuck Wayne or Box Hop Ju Sul Association. Anyone who violates this agreement has done so unauthorized and unlawful and will not hold Chuck Wayne or the Box Hop Ju Sul! Association responsible for any condition he was not aware of at the time. Chuck Wayne or the Box Hop Ju Sul Association has the right to terminate any students contract, studies, or memberships at any time without warning for any violation of this agreement. Any student uses his training unlawful outside of this school will be terminated. Chuck Wayne teaches self-defense and does not promote violent acts, teaches to kill, or mangle any person without reasonable cause. This training is based on self-defense. No refunds will be given once service has been rendered. I HAVE READ AND I UNDERSTAND THIS AGREEMENT APPLICATION, AND CHUCK WAYNE'S POLICY. I FIND MYSELF IN NO OBLIGATION TO ACCEPT THIS AGREEMENT IF I WISH NOT TO. I AGREE TO THIS APPLICATION AND I WISH TO BECOME A MEMBER OF THE CHUCK WAYNE PROGRAM OR BOX HAP JU SUL ACCOSIATION. |
